Notification Instructions not Clear

Was notified that a new credit card had been applied for in my name for Macy's. It was not me. It was not my wife. But the questions that you are asked to confirm ask if you opened up reward plan in last 60 days - I did, but not with Macy's, but because I responded yes, it dismissed the claim and is not investigating it now. The questions did not specify Macy's specifically.
